What board certification actually means

"Board-certified" shows up on a lot of provider profiles and ads, and it sounds important. It is — but not always in the way people assume, and not equally for every situation. Here's what it actually means.

Licensed and certified are two different things

A medical license is the legal minimum — it's what lets someone practice at all, granted by a state board after medical school and exams. Board certification is voluntary and additional. It means a physician went further: completing residency in a specialty and passing rigorous exams from a certifying board, like the member boards of the American Board of Medical Specialties (ABMS) for MDs, or the osteopathic equivalents for DOs. Licensed means "allowed to practice." Certified means "demonstrated specialty expertise."

What it signals

Certification tells you a doctor met a national standard in their specialty and, in most modern programs, keeps current through ongoing education and recertification. For anything where depth matters — surgery, oncology, cardiology, anesthesia — it's a credential worth checking, and you can verify it free at certificationmatters.org.

What it doesn't tell you

It isn't a guarantee of good bedside manner, and its absence isn't proof of a bad doctor. Plenty of excellent, fully licensed physicians practice without being certified in every area, especially in primary care. Certification also says nothing about whether someone takes your insurance or listens well. Treat it as one strong input, not the whole decision.

A reasonable rule

For routine primary care, an active license and a provider you trust is usually enough. For high-stakes or highly specialized care, look for board certification in the specialty that actually matters — not just any board. It's a quick check that tells you something real.
